In my city (Aurora Colorado) we don't add fluoride because it's in the water supply from the mountains at just around the recommended level. That is true in a lot of the Denver metro area.
Colorado Springs actually removes some fluoride from their water because naturally it's there in super high amounts. The Springs is actually the reason we found out the effects of fluoride.
