Originally part of the reasoning to remove the Shah of Iran in 1979 is that it was a hereditary office. Naming the son the heir of the father is rather counter to that, and the old guy didn't really want this. I rather suspect that the IRGC have been effectively running Iran for some time now and don't really give a damn about the Ayatollah. This makes the current one being in a coma a bit of a bonus, too. That's just my read on it though.

Killing him could make things worse. Right now he’s unable to rule so there are people behind the scenes who have to make decisions instead. I assume there will be a lot of politics, backstabbing, game of thrones struggle for power. But as long as Khamenei is still the supreme leader, there’s uncertainty…what if he wakes up? Kill him and one of the factions gets to install their own supreme leader, who’s very much alert and now in full control.
