I don’t think it’ll ever happen. NK is beholden to China, and I don’t think China wants to lose a major trade partner.

china won't allow it unless the US loses it's military power projection abilities. NK won't upset china. they'd kill millions of civilians in the opening salvo of artillery for sure, especially if they went nuclear, but without china's approval the south and US combined forces would push them out of the south and would likely end up with a unified korea under the south if china didn't intervene and then all bets are off. the south wants diplomatic reunification so they won't attack the north so nothing to worry about there unless they elect their own version of trump.

Mostly probably pollution from whatever fuel it had left, idk if they use warheads, if they do than whatever damage the warheads does on top of that. But realistically, not much. We used to blow up nukes in the ocean, torpedos and bombs
